What Are The Top 5 Important Factors When Picking A Host?

I am in need of a little input from everyone.

I need to know what people consider the top 5 most important factors are when deciding what host to go for, I know that space, bandwidth and cost are major factors but apart from them I am interested in any other factors.

The package would be for a shared Linux package with any company for a standard personal website (the basic package).

I am thinking:

Database qty (MySQL)
Email addresses (number of)
Ftp users (number of)
Sub domains (number of)
PHP (If supported what version)

These are what I would consider and I wonder if anyone would consider any other factors or put these in a different order of importance.

The reason I am asking is I need to create a formula to determine the most suitable host for a small to medium website and I want to base it on a number of factors (not sure how many yet nor their weight in the decision).

The outcome of this thread may well help people decide also!
